The Client Services Agreement

Every person entering into service with MirkaCare Services must have a Client Services Agreement (legal contract) in place to outline living and service expectations within 45 days after services start.

The Client Tenancy and Accommodation Agreement

If the individual is living in a MirkaCare-owned property, the client and/or guardian must at the same time agree to the terms of the Client Tenancy and Accommodation Agreement.  The individualized Client Tenancy and Accommodation Agreement outlines living and accommodation requirements, the room and board costs, and any additional services requiring additional fees such as food for a special diet, transportation services, special laundry services, additional phone/internet/digital television lines, or other things as spelled out as part of this agreement.

The Support Services and Care Plan

The Support Services and Care Plan addresses the many physical, medical, mental, developmental, spiritual, family dynamic and social dynamic dimensions of an individual’s care and developmental makeup. It is a comprehensive document that includes the client’s personal data, legal guardianship and emergency contact information, relevant history, individual service plan needs or requirements, care needs or requirements, any special diet requirements, physician information, medication administration instructions, transportation/mobility needs, social/recreational interests, personality, values, and client financial management instructions. The individual and their guardian are at the centre of and drive this process.

river-valley-in-winterAssessments are required to develop the initial Support Services and Care Plan. The assessment is performed by trained and qualified staff. They consist of the RN; staff trained to conduct behavioral assessments; executive management working in conjunction with health care professionals; and the individuals or their community-based care and services team, including case managers, doctors, therapists, pharmacists etc. MirkaCare uses the instruments and forms required by the group making the referral, as well as our own forms.

Each agreement must be reviewed and renewed annually, ideally at funding renewal time but within one year of receiving services. If the needs or services change at any time, the agreement will be updated.
